Tutor Application Forms

New Hire Packet

New tutors need to complete and return each form to the America Reads office (NRN 365). Tutors will not be considered as hired until each form has been completed fully and returned to the office. You can download each form individually or the entire packet containing all forms below.

Individual Forms

In addition to the above forms, new employees must provide 2 copies of their driver's license and social security card. We also need an original work permit.

Additional forms for New and Returning Tutors

The following forms are required for new and returning tutors each semester. These forms are also included in the entire packet download listed above.

• Tutor Information Sheet (PDF) (Word): Tutor contact information must be updated each semester. Download and return to the America Reads office.
• Work Schedule (PDF): Please indicate the times you are available to work. In addition, please provide a copy of your class schedule in weekly planner format.
• Hours Permitted to Work (PDF) (Word): This sheet calculates an estimate of the hours you are permitted to work each week based on the amount of money you have been given through Work Study.
